Our social navigation service is all about connecting people and whānau with the health and social services they need. We want to support them in addressing the underlying factors affecting their well-being, so they can see a better and lasting change in their lives.

Our navigators are here to assist you and your family (whānau) in accessing services and providing support during hospital or outpatient clinic visits. They will connect you with trusted community services that align with high-quality, culturally appropriate care. If you ever feel unheard or misunderstood, your navigators will act as advocates on your behalf.

Working collaboratively with your navigator, you’ll develop care plans and goals that are created, centered, and led by your family (whānau). This collaborative approach aims to empower you to self-manage your health journey with comprehensive support.

WE+ is a collective of like-minded healthcare organisations who choose to work together to deliver care. We are proud to be working with them to serve our community.
